Health Record Update

Health Record Update Form

 

Each year, every returning LPS student must have a Health Update Form completed by their parents. This form also gives consent for the nurse to administer Tylenol. (Link for forms is to the right.)  If your child will be taking medication other than Tylenol at school, please also complete a Medication Permission Form. If your child has a medical diagnosis of Asthma, Diabetes, Seizures, or a Severe Food Allergy please complete an Action Plan and obtain a physician signature.

Required Immunization

All incoming 12th graders are required to receive a Meningococcal Conjugate immunization after the age of 16.  Documentation needs to be turned in to the school nurse before the first day of school.

 

 

State immunization requirements have changed for the 2016-2017 school year.

Influenza

We are beginning to see influenza in the community.  Please remember that students need to be fever free for 24 hours without the use of fever reducing medication before they can return to school.  If your child has a positive influenza test please call the school and report this to the nurse.  We collect date and report it to Clay County Health Center.  (No personal information is included in this data.)  You can reach the LHS nurses at 736-7040.
